Why implant position decides how the smile will look
The surgeon places the implant where there is bone. The prosthodontist makes a crown that has to look right. All the difficulty of the work lies between those two tasks.
After a tooth is lost, bone resorbs unevenly: the outer wall, on the lip side, loses volume faster than the inner one. Within a year or two the ridge becomes thinner and its thicker part shifts inwards, towards the palate or the tongue.
The implant goes where there is enough bone for reliable fixation — often into that inner part. Sometimes the opposite happens and the axis naturally exits outwards. Either way, the direction of the implant does not match how the future tooth should stand.
Why this matters for appearance
If you simply continue the implant axis upwards, the crown comes out rotated — it will protrude or, conversely, sit further in than the neighbouring teeth. In the smile zone this shows immediately: the tooth is "out of line", the gum around it looks different, the shape differs from its neighbours.
What the dentist does
- plans the implant position in advance, starting from the future crown rather than from the bone
- grafts bone on the outer side where volume is lacking, so the implant can go in at the correct axis
- uses a custom or angled abutment to turn the crown into the right position
- chooses between screw and cement retention precisely on the basis of that angle
What depends on more than the tooth
How high the lip rises in a smile matters too. Some patients show only teeth, others show gum as well. In the second case the requirements for implant position and gum contour are noticeably stricter, because there is nothing to hide an inaccuracy behind.
